Rudolf Ekstein studied psychology, philosophy and history at the University of Vienna. Among his teachers were Anna Freud, Willi Hoffer and August Aichhorn. He was promoted in 1937. He was Jewish and after the Germans entered Austria in 1938 he fled to the USA. In Boston he was futher educated as a psychoanalist by Eduard Hitschmann and he was promoted as second time. From 1947 to 1957 he worked for the Menninger Foundation in Topeka, Kansas. From 1957 to 1976 he was employed at the Reiss Davis Child Study Center in Los Angeles.

He became well known for his work with phychotic and autistic children. Between 1960 and 1996 he frequently visited Vienna where he gave guest lectures at the university. He died in 2005 in Los Angeles. His ashes were buried at the Feuerhalle Simmering in Vienna.
